SR Forms

Can someone point me in the right direction for SR forms? I tried google and I went to copyright.gov

Copyright.gov doesnt have those available for download and I dont know how to order one. I think they changed the forms around or something along those lines.

I went back to copyright.gov and learned that CO forms have replaced the SRs. I have also found a way to request them straight from the site.
